Please provide a sample of the vinyl siding of a minimum length of 3 feet (0.91 m) for each wall where the problem related to the claim has been observed.

The origin of each sample must be legibly identified on the back of the sample (example: back wall). Also be sure to have the lot number on each sample taken. If it is not already engraved on the sample taken, please find it on another panel and write it on the back of the sample taken.

Note that it is important to keep the original length of the sample taken (do not cut the sample taken). In this way, you will be able to put it back on the wall in case the claim is not allowed.

Once we have received and analyzed the samples, we will be able to determine the next steps. Samples may be returned upon request for any claim that is not justified.

Important: Samples must be received by Duchesne within fourteen business days of the date of submission of the claim, failing which the request will be automatically canceled.

Please provide a sample of the product in connection with the claim. This sample must demonstrate the problem observed.

If possible, please also provide the lot number of the product on the carton or on the product label.
